Sladjana's Story

In need of 24 hour care, the transition to Nulsen has been a happy one for Sladjana.

Hitting adulthood means freedom, and Sladjana loves to get out and about to concerts, restaurants, nightclubs, and to see her favourite Perth Wildcats. Her medical needs mean she requires 24 hour direct care, and the Nulsen team have stepped up to deliver.

Upon turning 18, Sladjana transitioned from children’s services to a community-based model of care at Nulsen Disability Services. With the need for tracheostomy and ventilator care, Nulsen did a lot of research and had a lot of conversations before they could commit.

To ensure everything was successful, the team rostered extensive buddy shifts to become familiar with the care that was required during a complex transition.


We do a planning roster every month with Perth Home Care, so then I can choose my outings that I want to do and how many hours each day and when I want other hours.

Coordinator of Person Centred Planning at Nulsen, Suzie Hoffman worked with Sladjana full time for 4 months to gain a strong insight into what was important to her, how we could best support her day to day and emotional needs.

“Everybody looked outside the box and reassessed what their usual service delivery parameters were and really came to the party to make things work.

“We pushed the envelope and we broke down barriers and we went out of our comfort zone and into a new territory but I’m so glad we’ve done it because it’s a huge success story.”
